What to Expect During Your First Appointment.

INTAKE APPOINTMENT

Before your first Visit: You will receive an email from us containing all intake paperwork through our Electronic Health Records program. Please read through the information and fill out the paperwork prior to your first session. If you are unable to complete the intake paperwork online please schedule an appointment with our clinical case manager for further assistance. It will save us time and allow us to start the therapeutic process.

If you prefer tangible documents for intakes please let our clinical case manager know. Thank You.

Out of Network Benefits

We can provide documentation for you to submit for “out of network” coverage. If you have met your deductible you may be covered to receive “out of network” benefits. We will gladly bill most major insurance companies if you have a PPO or a POS plan. We are not able to bill out-of network benefits for any HMO plans. Please give us a call for more information.

Cancellation Policy

If you are unable to attend a scheduled appointment, we ask that you provide at least 24 hours’ notice to our office or your therapist. Because this appointment time is reserved specifically for you, cancellations made with less than 24 hours’ notice or missed appointments prevent us from offering that time to another client.

Appointments cancelled with less than 24 hours’ notice, or appointments that are missed without notice, will be charged the full session fee unless the cancellation is due to a contagious illness or an unforeseen emergency. If you are experiencing symptoms of a contagious illness (such as fever, vomiting, diarrhea, flu-like symptoms, COVID-19, or another condition that could be transmitted to others), we ask that you notify us as soon as possible and stay home. In these situations, the late cancellation fee will be waived.
