Before your first Visit: You will receive an email from us containing all intake paperwork through our Electronic Health Records program. Please read through the information and fill out the paperwork prior to your first session. If you are unable to complete the intake paperwork online please schedule an appointment with our clinical case manager for further assistance. It will save us time and allow us to start the therapeutic process.

We can provide documentation for you to submit for “out of network” coverage. If you have met your deductible you may be covered to receive “out of network” benefits. We will gladly bill most major insurance companies if you have a PPO or a POS plan. We are not able to bill out-of network benefits for any HMO plans. Please give us a call for more information.
