We can provide documentation for you to submit for "out of network" coverage.
If you have met your deductible you may be covered to receive "out of network" benefits. We will gladly bill most major insurance companies if you have a PPO or a POS plan. We are not able to bill out-of network benefits for any HMO plans.

If you are unable to attend an appointment, we request that you provide at least 24 hours advanced notice to our office/ your scheduled therapist. Since we are unable to use this time for another client, please note that you will be billed for the entire cost of your scheduled appointment if it is not timely cancelled, unless such cancellation is due to illness or an emergency.
For cancellations made with less than 24 hour notice (unless due to illness or an emergency) or a scheduled appointment that is completely missed, you will be mailed a bill directly for the full session fee.

Before your first Visit: You will receive an email from us containing all intake paperwork through our Electronic Health Records program. Please read through the information and fill out the paperwork prior to your first session. If you are unable to complete the intake paperwork online please schedule an appointment with our clinical case manager for further assistance. It will save us time and allow us to start the therapeutic process.
If you prefer tangible documents for intakes please let our clinical case manager know. Thank You.
